Professional / School Individual Referral

Use this form to refer one young person who needs individual support from UTurn outside an already agreed school programme.

Before you start

It helps to have:

  • the young person’s basic and education details
  • parent/carer details where appropriate
  • the reason for referral and what support may help
  • details of any support already in place
  • relevant medical, additional-needs or safeguarding information

UTurn is not an emergency service. If a young person is in immediate danger, call 999. Urgent safeguarding concerns should also follow your organisation’s safeguarding procedures.
