Agreed School Programme Referral
Use this form for one or more pupils joining a project or programme already agreed with UTurn.
Add one pupil, or include all selected pupils together using Add Another Young Person. There is no spreadsheet to download or return.

UTurn has already agreed a project or programme with your school and you are referring one or more selected pupils.
Need Individual Support Instead?
If one young person needs support outside an already agreed school programme, use our Individual Support Referral instead.
No separate full registration is normally required. This Programme Referral provides the programme, safety, emergency-contact and permission information needed for young people attending only the agreed school mentoring programme. A full registration is only needed if UTurn requests one or the young person also joins clubs, Holiday Club, trips or other activities outside the programme.

Clear Arrangements for Every Young Person
Some young people may be collected from school by the UTurn minibus, returned to school, collected from UTurn or leave independently following an agreed arrangement.
Please make sure the correct travel and end-of-session arrangement is recorded for each young person before submitting the referral.
Independent departure during the school day
If a young person will leave UTurn independently during the school day, the school must confirm that this is the agreed arrangement and that the appropriate parent/carer permission has been obtained.

If anything is missing or needs clarification, we will normally contact the named school contact.

Programme Arrangements
Once the referrals and arrangements are in place, delivery can begin as agreed with the school.
