Parent/carer details, emergency contacts, collection arrangements and leaving permissions.

Medical conditions, allergies, medication, dietary needs and relevant permissions.

Registration Is Not a Booking
Completing this form registers the young person with UTurn. Activities, Holiday Clubs, trips and other bookable sessions still need to be booked separately where required.

Use this form when a young person will be attending UTurn activities, Holiday Club, trips, mentoring, transport or ongoing support.
Referral
If you are specifically looking for mentoring or targeted support, please use the appropriate referral form as well.

Funding & Family Membership
UTurn may have funded activities, Holiday Club places or programmes available at different times throughout the year.
The registration form asks about Free School Meals and low income so we can help identify families who may be eligible when funded opportunities are available.
Family Membership is optional. It is not required to access an eligible funded place.
